Last week I had to stop at the doctor's office to get my older son's working papers completed--a doctor's signature was required on the form. 
When I called ahead, the office staff told me to bring it over and it would be taken care of quickly.
But by the time I reached the doctor's office, the waiting room was a zoo. There were no empty chairs, and I had to wait ten minutes for someone to take the form. Clearly the doctor's signature was not going to happen. They told me to come back the next morning to pick up the paper. And when I returned, the same scene awaited me. There was quite a lineup of parents picking up forms: working papers, schools, and athletics all have their unique forms that must be filled out.
I wish that, after the yearly physical exam takes place, the doctor could give out a standard, signed form that could be copied and attached to all of these other things. It would save parents, and the doctors' offices, a lot of time and trouble. (And at $10 per form that's not filled out at the yearly physical, it would save money too!)

I must be spoiled (lucky)? because our pediatrian does provide this type of form, its good for one year. We use it for daycare and school!
Wait til high school, where the form HAS to be provided by the school.
